上颌尖牙牙内陷伴牙龈瘤患者的多学科联合治疗1例

Multidisciplinary Combined Treatment of Maxillary Canine with Dens Invaginatus and Epulis: A Case Report

摘要: 目的:牙内陷伴牙龈瘤是一种罕见病,本文报道1例因上颌尖牙牙内陷伴牙龈瘤的多学科联合治疗的病例。诊治经过:在明确诊断及患者知情同意后,对患牙进行多学科联合治疗:包括全口牙周基础治疗、根管治疗、牙龈瘤切除术、根尖切除术;炎症控制后进一步行结缔组织移植术进行牙龈软组织增量以及口腔修复治疗恢复美观。结果:牙龈瘤切除术后两年半内,未见牙龈瘤复发;患牙无叩痛,未及窦道;唇侧牙龈健康,未见明显退缩;口腔修复体临床效果稳定。结论:其治疗的关键在于早发现,同时结合牙内陷的类型、牙髓感染及牙周破坏的程度等选择合适的治疗方案。

Abstract: Objective: This article reported a case of multidisciplinary combined treatment of epulis due to dens invaginatus. Diagnosis and treatment: After clear diagnosis and informed consent, the patient underwent multidisciplinary treatment, including periodontal initial therapy and root canal treatment, followed by epulis excision and apical surgical resection; after inflammation control, connective tissue grafting was carried out for gingival soft tissue augmentation and prosthodontic treatment to restore the aesthetics. Results: Two and a half years after epulis resection, no recurrence of epulis; no percussion pain in the tooth, no sinus tract; no obvious recession of the labial gingiva, no obvious periodontitis, and stable clinical results of the restoration. Conclusions: Dens invaginatus is a rare disease, for the treatment of dens invaginatus, the key lies in early detection, and at the same time combining with the type of invagination, pulpal infection and the degree of periodontal destruction to choose the appropriate treatment programs.
