近中及水平位阻生下颌第三磨牙微创拔除分牙方法研究进展

Research Progress on the Method of teeth sectioning for Minimally Invasive Extraction of Mesial and Horizontal Impacted Mandibular Third Molars

摘要: 作为口腔颌面外科门诊最常见的手术操作,下颌阻生第三磨牙拔除术的核心在于有效解除邻牙、骨组织及软组织的阻力。其中近中及水平阻生是最常见的阻生类型,其特殊的解剖位置常导致邻牙阻力与骨阻力并存。自劈凿法至高速涡轮手机技术,再到超声骨刀等新型器械的应用,分牙方法始终是去除阻力的关键。本文就下颌阻生第三磨牙微创拔除的分牙方法与技巧作一综述,旨在提高拔牙效率,为口腔颌面外科医师提供参考。

Abstract: As the most common surgical operation in oral surgery clinics, the core of mandibular third molar extraction is to effectively relieve the resistance of adjacent teeth, bone and soft tissues. Mesial and horizontal impaction, the most prevalent form of impaction, frequently results in the coexistence of adjacent teeth and bone resistance due to their anatomical position. The evolution of surgical instruments, from the splitting chisel method to high-speed turbine handpiece technology, and the subsequent development of novel instruments such as ultrasonic bone knives, has underscored the pivotal role of the sectioning method in overcoming resistance. This article reviews the sectioning methods and techniques in minimally invasive extraction of mandibular third molars, with a view to improving extraction efficiency and serving as a reference for clinicians.
