摘要: 药物相关性颌骨坏死(medication-related osteonecrosis of the jaw,MRONJ)是一种主要与抗骨吸收和抗血管生成等药物使用有关的发生于颌骨的严重并发症。本文描述了MRONJ的发病机制、危险因素、诊断方法及防治策略的最新研究进展。MRONJ的发病机制涉及药物对骨代谢的影响、血管生成抑制、局部感染和免疫反应等多个方面。早期诊断和预防对于改善预后至关重要。其治疗以手术治疗为主,新兴治疗方法如己酮可可碱联合α-生育酚的药物协同疗法和新技术辅助手术,但未来仍需进一步开展大规模临床研究以优化防治策略。

Abstract: Medication-related osteonecrosis of the jaw(MRONJ)is a serious complication that occurs in the jaw and is mainly related to the use of drugs such as antiresorptive and antiangiogenic drugs. This article reviews the latest research progress on the pathogenesis, risk factors, diagnostic methods, and prevention strategies of MRONJ. The pathogenesis of MRONJ involves the effects of drugs on bone metabolism, angiogenesis inhibition, local infection and immune response. Early diagnosis and prevention are essential to improve outcomes. At present, the treatment of MRONJ is mainly surgical, and there are also some emerging treatments, such as the synergistic therapy of pentoxifylline+α-tocopherol and the application of new technologies to assist in surgical treatment. Large-scale clinical studies are needed in the future to optimize the prevention and treatment strategies of MRONJ.
