摘要: 目的:探讨浓缩生长因子(concentrated growth factor,CGF)对下颌低位埋伏阻生第三磨牙拔除后并发症的缓解及软硬组织愈合修复的影响。方法:选取50例双侧对称埋伏下颌阻生第三磨牙患者随机分为试验组与对照组,拔牙术后使用CGF的为试验组,常规拔牙的为对照组。对比术后第1、第3、第7天疼痛、肿胀、软组织愈合等术后并发症,及术后3个月第二磨牙牙周袋和拔牙创愈合情况。结果:术后7天试验组创口愈合指数优于对照组(P<0.01)。术后1、3天试验组较对照组局部肿胀缓解明显(P<0.001),第7天时两组未见明显差异。术后第1天视觉模拟评分无统计学差异(P=0.250),术后第3、7天试验组疼痛评分低于对照组(P=0.001)。试验组与对照组牙槽嵴宽度及颊舌侧骨壁高度对比具有统计学差异(P<0.001)。试验组骨密度显著高于对照组。试验组牙周袋改善情况具有显著性差异(P<0.001)。结论:下颌低位埋伏第三磨牙拔除术后局部应用CGF可以有效促进伤口愈合、缓解局部肿胀、疼痛等并发症并促进组织再生。

Abstract: Objective: To investigate the effect of concentrated growth factor (CGF) on the relief of complications and the healing and repair of soft and hard tissue after extraction of lower mandibular impacted third molar. Methods: Fifty patients with lower impacted third molars were randomly divided into experimental group and control group. Postoperative complications pain, swelling and soft tissue healing were compared at 1, 3 and 7 days after surgery, and the second molar pocket and alveolar bone tissue healing were compared 3 months later. Results: The wound healing index of experimental group was better than that of control group 7 days after operation (P<0.01). CGF alleviated local swelling 1 and 3 days after operation (P<0.001), and there was no significant difference between CGF and CGF on day 7. There was no statistical significance in visual simulation scores on the first day after surgery (P=0.250), and experimental group were lower than those on the control group on the 3rd and 7th day after surgery (P=0.001). There were significant differences in alveolar ridge width and buccal-lingual wall height between experimental group and control group (P<0.001). Bone density in experimental group was significantly higher than that in control group. The improvement of periodontal pocket in experimental group was significantly different (P<0.001). Conclusions: Local application of CGF can effectively promote wound healing, relieve local swelling and pain, and promote tissue regeneration after the extraction of lower mandibular third molar.
