城市居民义齿修复方式临床决策偏好研究

摘要/Abstract

摘要： 目的本研究通过设计离散选择模型及问卷,调研我国一线城市居民对义齿修复方式的选择偏好并分析其影响因素。方法通过文献综述、专家咨询、预实验等确定离散选择模型的属性与水平,运用Ngene软件、固定效应d-efficient模型设计调查问卷。经预实验测试后,抽取北京、上海、广州、深圳市的85位居民进行网络调研,使用STATA 15.0软件中的无序多分类logit模型对数据进行统计分析。结果在牙列缺损的假设条件下,城市居民选择义齿修复方式时不同属性的偏好效用从大到小依次为：5年义齿存留率（β=-2.079）、咀嚼功能（β=1.319）、美观程度（β=0.963）、固定/活动（可摘戴）（β=-0.653）和治疗过程复杂程度（β=-0.226）,治疗费用（β=0.000）在本试验中未影响居民的选择。亚组分析表明,教育程度较高、收入水平较高的居民更关注美观程度,教育程度较低的居民更关注咀嚼功能,有过缺牙或牙齿修复经历的居民更关注5年义齿存留率。结论 5年义齿存留率和咀嚼功能是我国一线城市居民最偏好的属性,其次是美观程度。为提供“以患者为中心”的健康管理,医护人员提供临床建议时应考虑患者的社会人口学特征并详细解释不同修复方式中的不同属性及其水平。
