摘要: 目的:本文旨在探索伴龈乳头缺损的牙龈退缩分类3型(recession type 3,RT3类)的诊治思路和临床经验。诊治经过:右上前牙区伴龈乳头缺损的牙龈退缩3类的年轻女性患者,考虑到应用常规的根面覆盖手术难以获得理想的美学效果,为其设计了基于结缔组织移植的软组织手术同期进行龈乳头重建及根面覆盖。结果:经1次手术达到了改善黑三角及获得部分根面覆盖的诊治目标,患者对当前治疗效果表示满意。结论:本病例验证了通过龈乳头重建而改善RT3类牙龈退缩的可能性。

Abstract: Objective: This article aims to explore the diagnostic rationale and clinical management strategies for RT3 gingival recession (GR) with concomitant gingival papilla deficiency. Diagnosis and treatment: The case invovled a young female patient presenting with RT3 recession and papilla deficiency in the right maxillary anterior region. Considering the challenge of achieving optimal aesthetic outcomes through conventional root coverage procedures, a connective tissue graft-based soft tissue surgery was designed and performed, simultaneously addressing both papilla reconstruction and root coverage. Results: Through a single surgical intervention, partial root coverage was achieved with significant improvement in black triangle morphology. The patient expressed satisfaction with both functional and esthetic outcomes. Conclusions: This case validates the potential for improving RT3 GR through gingival papilla reconstruction, demonstrating that coordinated soft tissue augmentation can partially overcome the limitations of conventional root coverage techniques in complex GR scenarios.
