摘要: 目的:探讨情景模拟联合以案例为基础的教学模式(case-based learning,CBL)对提高口腔全科住培医师医患沟通能力的作用。方法:本研究选取8例既往的典型病例及纠纷病例,设计医患沟通场景,构建可用于口腔医患沟通的情景模拟案例库。以2023年9月至2024年8月进入北京大学口腔医院综合治疗二科实习的17名住培医师为研究对象,由2名不参与教学过程的教师采用《医患沟通技能评价表》评价学生接诊时的医患沟通能力。在完成情景模拟联合CBL 教学后,仍由这2名教师再次采用量表评分。同时对住培医师进行问卷调查评价教学课程。结果:17名住培医师的量表初始评价总分为(18.00±2.29),经过课程培训后总分升至(22.41±1.54),差异具有统计学意义(P<0.05)。多数学生认为课程设置必要性高、参与度好、提升感强。结论:情景模拟联合CBL 教学可提升住培医师课程参与度、提高医患沟通能力。

Abstract: Objective: To evaluate the effect of scenario simulation combined with case-based learning (CBL) in improving doctor-patient communication skills of residents of general dentistry. Methods: This study selected eight typical and disputed clinical cases to design physician-patient communication scenarios, establishing a simulated case database for dental communication training. The research was carried out on 17 residents of general dentistry from Department of General DentistryⅡ, Peking University School and Hospital of Stomatology from September 2023 to August 2024. After completing the theoretical knowledge of doctor-patient communication, the students’ doctor-patient communication abilities were evaluated by the teachers using the set elicit give understand end (SEGUE) framework. After the completion of the scenario simulation combined with CBL teaching, the teachers evaluated again. At the same time, students were asked to evaluate the course by questionnaire. Results: The total score of the initial evaluation of the scale was (18.00±2.29), and the total score increased to (22.41±1.54) after the training, and the difference was statistically significant (P<0.05). The scores of the residents in the preparation stage, information collection stage, understanding the patient stage, and the end of the inquiry stage were all improved statistically. Most of the students thought that the course was necessary, and they had a strong sense of involvement and promotion. Conclusion: Scenario simulation combined with CBL teaching mode can improve the participation and the communication skills of the residents in the standardized training of general dentistry, and can be popularized and applied in the training base in the future.
