Oral Lichenoid Lesions of Chronic Graft-Versus-Host Disease: A Case Report

摘要: 目的:报道1例慢性移植物抗宿主病相关口腔苔藓样损害的诊疗经验,探讨其临床特点与个体化治疗策略。诊治经过:患者行骨髓移植术后,口腔黏膜出现白纹伴迁延难愈的大面积充血糜烂。为规避全身用药对基础疾病及肝肾功能的影响,为其制定并实施个体化的分阶段局部药物治疗方案。结果:经多阶段局部综合治疗,口内糜烂面痊愈,白纹明显减退,疼痛消失,实现临床愈合。结论:慢性移植物抗宿主病相关口腔苔藓样损害是口腔潜在恶性疾患,对伴有全身性疾病、用药复杂的患者,以局部治疗为核心的个体化方案,能有效促进病损愈合,且安全性高,具有重要的临床借鉴意义。

Abstract: Objective: This case reported the diagnosis and treatment for oral lichenoid lesions of chronic graft-versus-host disease, aiming to explore its clinical characteristics and individualized treatment strategies. Diagnosis and Treatment: After bone marrow transplantation, the patient developed white reticular striae in the oral mucosa accompanied by refractory large atrophic and erosive lesions. To avoid the impact of systemic medication on the underlying disease and hepatorenal function, we developed and implemented an individualized and staged topical treatment strategy. Results: Through multi-stage local comprehensive treatment, the atrophic and erosive lesions healed, the white reticular striae relieved significantly, and the pain alleviated completely. Conclusions: Oral lichenoid lesions of chronic graft-versus-host disease is a potential malignant oral disease. This case indicated that for patients with systemic diseases and complex medication regimen, an individualized treatment strategy with primarily topical therapy could effectively promote the healing of the lesions with safety, which has important clinical reference significance.
