Adult Hyperdivergent Skeletal ClassⅡMalocclusion with Open Bite Treated by Clear Aligners Combined with Miniscrews: A Case Report

Abstract: Objective: In this case, an adult with skeletal ClassⅡ malocclusion and open bite was treated with clear aligners in combination with miniscrews and tongue spurs. Diagnosis and treatment: An adult female with skeletal ClassⅡrelationship, molar ClassⅠ relationship, hyperdivergent profile and anterior open bite was treated with clear aligners. The miniscrews were implanted in the molar regions for molar intrusion. The tongue spurs were placed in the anterior teeth to correct tongue thrusting habit. Results: After 26 months of orthodontic treatment, the open bite was closed, and bilateral ClassⅠmolar/canine relationship, dental midline consistency were achieved with optimal root parallelism, reduced ANB and mandibular plane angle. The lateral facial esthetics was improved as well. These treatment results were proved to be stable after six months of retention. Conclusions: Clear aligners combined with miniscrews for molar intrusion can provide effective vertical control during the treatment of skeletal Class Ⅱ malocclusion with a hyperdivergent profile and open bite. Tongue spurs are effective in correcting tongue spitting habits at the same time.
