Abstract: Objective: This article presented the multidisciplinary management of a young patient with generalized Stage IV Grade C periodontitis, offering a novel approach to the treatment of severe and complex periodontitis patients. Diagnosis and treatment: A 20-year-old male patient diagnosed with generalized StageⅣ Grade C periodontitis, of which included the loss of multiple functional teeth, pathological tooth migration, and secondary occlusal trauma. A systematic therapeutic protocol termed inflammation control-occlusal unloading-functional reconstruction was implemented, integrating periodontal treatment with early occlusal intervention using interim removable dentures, followed by orthodontic alignment and subsequent implant restoration. Results: The multidisciplinary treatment strategy successfully restored periodontal health, resolved occlusal trauma, and reestablished a functional dentition, leading to a favorable clinical outcome. Conclusions: The staged treatment system of inflammation control-bite unloading-functional reconstruction can achieve improved bite function, periodontal stability, and aesthetic restoration effects.
