    Discussion on Key Points of Standardized Construction of Oral Emergency Department

    In recent years, with the rapid development of oral emergency, medical institutions generally set up oral emergency department, but the level of the construction of different medical institutions of oral emergency department is uneven. The standards for the construction of oral emergency department are unclear. In this paper, the key points of standardized construction of the oral emergency department will be discussed from the responsibility and business scope of oral emergency, the spatial layout of the oral emergency room, the allocation of equipment and first aid drugs, the cultivat of professional talent team and emergency treatment ability, the informatization of diagnosis, treatment and management, so as to promote and improve the construction level of the oral emergency department.

    With the deepening of the research on sports mouthguard, there are currently three different types of sports mouthguard classification. In order to improve the protective effect of sports mouthguard, researchers have continuously improved the manufacturing materials and design, and strengthened the use and maintenance of sports mouthguard. In recent years, the role of doctors in the protection of sports mouthguard has also been paid attention to. This study introduces the research status of sports mouthguard at home and abroad, and summarizes the types, protection mechanisms and effects, manufacturing technologyies maintenance methods and stomatologists participation of sports mouthguards, in order to provide references for stomatologists in the design, production, maintenance and corresponding research of sports mouthguards.

    The Impact of Online Appointments in Dental Clinic on Emergency Patients

    Objective: To analyze the changes in clinical diagnosis and treatment characteristics of oral emergency patients before and after online appointments in dental clinics, in order to guide the clinical grading of oral emergency diagnosis and treatment. Methods: The clinical data of 130591 first-time patients who visited the Emergency Department of Peking University Stomatological Hospital in 2019 before the implementation of online appointment and 2021 after the implementation of online appointment were reviewed and analyzed respectively. The changes in age, gender, visit time, disease type and visit time of patients in oral emergency in two years were compared through Chi-squared test. Results: The total number of first diagnosed patients in 2021 increased compared to 2019, with an increase in the proportion of elderly patients and a decrease in the number of patients in adolescents, children, and adults, all showing statistical differences. The proportion of men and women in 2021 showed no significant difference compared to 2019, while the proportion and duration of non emergency patients in 2021 increased compared to 2019, showing statistical differences. Conclusions: The popularization of online medical care has brought convenience to patients, but we still need to explore and promote a graded diagnosis and treatment system, scientifically guide patients to seek medical treatment, avoid waste of medical resources, and build a more reasonable, scientific, and more responsive oral emergency management system.

    Oral and maxillofacial space infection is one of the common diseases in maxillofacial surgery. Infections usually accumulate in multiple maxillofacial Spaces and are mainly manifested as local redness, swelling, heat and pain, restricted mouth opening, swallowing discomfort and speech confusion, causing inconvenience to patients' daily life. If the infection cannot be controlled in time, it will lead to breathing difficulties, sepsis and even asphyxia risks, and in severe cases, the patients' lives will be endangered. Therefore, clinical attention is paid to the treatment of maxillofacial space infection. In Western medicine, antibiotics, abscess incision and drainage, microwave treatment, closed negative pressure drainage and other treatment methods have a certain effect. traditional Chinese medicine is extensive and profound, both internal and external treatment in the treatment of maxillofacial space infection has a good effect. This article reviews the treatment of maxillofacial space infection by integrated and western medicine to provide a new clinical idea for clinical treatment.

    Multidisciplinary Treatment of Complicated Dental Trauma of Upper Anterior Teeth: A Case Report

    Objective: This case presents a multidisciplinary treatment and one-year follow-up of complicated dental trauma occurred in upper anterior teeth, showing the diagnosis and treatment details at different stages of the dental trauma, hoping to provide reference for clinicians. Diagnosis and treatment: Female, 21 years old, visiting for the fracture and avulsion of upper anterior teeth. The surgeon performed multidisciplinary comprehensive treatment including tooth replantation and fixation, root canal treatment, modified crown lengthening surgery, fractured crown bonding, post and core crown restoration. Results: After 1 year follow-up, the crowns showed normal function with good marginal adaptation, stable gingival margin position and stable gingival papilla with no black triangle. Conclusions: Through multidisciplinary comprehensive diagnosis and treatment, ideal treatment results can be achieved for complicated crown-root fracture, which may maximizing the preservation of natural teeth.

    Objective: Traumatic dental injuries often result in crown fractures, the treatment of anterior tooth trauma accompanied by combined crown-root fractures is challenging, requiring consideration of periodontal, endodontic, orthodontic, and restorative factors. Proper diagnosis & multidisciplinary treatments are important for achieving good results. Diagnosis and treatment: This case report describes the co nservative management of a large cyst-like periapical lesion in the lateral incisors of the maxilla of a 19-year-old female with the chief complaint of slightly unwell of the anterior region with no history of pain and traumatic accident 5 years ago. After root canal retreatment, periapical surgery, orthodontic traction, and periodontal surgery, the injured teeth were repaired with veneers. Results: After 8 years of follow-up, the teeth were stable. Conclusions: For the complex crown-root combined fracture case, there is no consensus of extraction and retention treatment suggestions to preserve the injured teeth taking into account the aesthetics and functionality. It is very important for dentists to preserve the injured teeth.

    Primary Closure for Facial Injury Caused by Bear Scratch: A Case Report

    Objective: Animal bites and scratches are one of the most common traumatic injuries in the oral and maxillofacial regions. However, there is still controversy over whether primary closure should be performed on patients with animal bites and scratches in the oral and maxillofacial regions. To clarify the therapeutic effect of primary closure for facial animal scratches, this study will report one case of primary closure for bear scratches. Diagnosis and treatment: A 49-year-old male patient visited the hospital 20 hours after being scratched by a bear on the face. He had received tetanus, rabies immunoglobulin, and rabies vaccine 12 hours earlier. In this study the wound debridement and primary closure was performed immediately at the first visit. Wound infection and aesthetic recovery were recorded during the follow-up. Results: The results showed that the patient underwent primary closure had no postoperative wound infection and got satisfactory scar recovery.Conclusions: For patients with facial animal scratches that can be thoroughly debridement, primary closure can improve postoperative aesthetic function without increasing wound infection rate, thereby reducing the appearance, physiological, and psychological trauma caused by the trauma to the patient.

    One Case of Retreatment of Upper Anterior Tooth Trauma after Incorrect Diaplasis

    Objective: Good therapeutic results were obtained by retreament and fixation of upper anterior tooth with incorrect diaplasis in other hospitals. Diagnosis and treatment: This paper reported a patient with upper anterior teeth trauma with incorrect diaplasis in other hospital. Based on the patient's medical history and findings of the clinical examinations and imaging examinations, the final diagnosis was obtained.The affected teeth were re-diaplasis and fixed which with the perfect root canal treatment.Finally, the satisfied clinical results were achieved. Results: A follow-up examination was performed 3、12、and 24 months after the treatment. Clinical examination found no obvious periodontal pockets, tooth discoloration, nor swelling of the gums around affected teeth. The radiographic images obtained during the follow-up examination showed that the periodontal lesions and root resorption images were gradually healed. Conclusions: The correct diagnosis, timely and correct treatment and follow-up are crucial to achieve a good prognosis of dental trauma.

    Objective: Patients with tongue defects after oral cancer and oropharyngeal cancer often have varying degrees of dysphagia, which seriously affects the quality of life of patients. Most patients eventually die due to concurrent aspiration pneumonia and cachexia. This research aims to find an effective treatment for dysphagia to solve this clinical problem. Diagnosis and treatment: In this case, the maxillary prosthetic device was used to reshape the shape of the hard palate to improve the contact between the tongue and the hard palate during swallowing. At the same time, combined with Shaker rehabilitation training, the strength of the remaining suprahyoid muscles was strengthened to improve the swallowing function of patients with tongue defects after oral cancer surgery. At the same time, the validity and feasibility of this method are evaluated. Results: The water swallow test was upgraded to level II, with a speech speed detection of 6±1, which basically reached the preoperative level. The overall range of tongue movement of the patient was significantly increased, and the flexibility was also significantly improved after 3 months of intervention. Conclusions: The combination of palatal augmentation prosthesis (PAP) and Shaker exercise can effectively improve the swallowing function of patients with tongue defects after tongue cancer surgery.

    Clinical Diagnosis and Treatment of Prolapse Dislocation of Young Permanent Teeth: a Case Report

    Objective: In this paper, we will report a case of maxillary permanent central incisor dislocation with lower lip contusion and laceration. Combined with the latest guidelines issued by the International Society for Dental Trauma in 2020, we will carry out standardized treatment for young patients with permanent tooth dislocation. Diagnosis and treatment: After debridement of the lower lip mucosa, the contusion and laceration of the lower lip mucosa was sutured under local anesthesia. Manual reduction of $\begin{array}{l|l}1 & 1 \\ \hline \end{array}$ dislocated teeth was carried out, then the neck of the anterior teeth and tear the gums were sutured. $\begin{array}{l|l}2 & 2 \\ \hline \end{array}$ had not yet sprouted, so $\begin{array}{l|l} Ⅴ\ Ⅳ\ Ⅲ\ 1& 1 \ Ⅲ\ Ⅳ\ Ⅴ\\ \hline \end{array}$ quartz fiber band fixation was performed. The occlusal relationship was examined after operation and the children and their families were advised to pay attention to the maintenance of oral hygiene. Results: Two weeks after operation, $\begin{array}{l|l}1 & 1 \\ \hline \end{array}$ crowns were intact, $1\rfloor$ was found with percussion pain (±), second degree loosening, $\lfloor 1 $ was found with percussion pain (–), second degree loosening. 8 weeks later, $\begin{array}{l|l}1 & 1 \\ \hline \end{array}$ were found withⅠ degree loosening, no discoloration of dental crown and good recovery of lower lip mucosa. At 6 months, 1 year and 2 years after operation, $\begin{array}{l|l}1 & 1 \\ \hline \end{array}$ were found with no pain, no loosening, no obvious change in crown color, normal pulp vitality test and good prognosis. Conclusions: For the cases of young permanent tooth dislocation, the standardized diagnosis and treatment with reference to the latest guidelines issued by the International Society for Dental Trauma is beneficial to the continued development of young permanent teeth. The standardized diagnosis and treatment of young permanent tooth dislocation can be used in oral emergency and outpatient service.

    Objective: The multidisciplinary treatment method and experience of a case of complicated dental trauma combined with alveolar socket wall fracture were reported. Diagnosis and treatment: The patient was treated with standardized sequential treatment including replantation and fixation of the injured teeth, alveolar bone augmentation surgery and implantation of concentrated growth factor. Results: After 1 month, 3 months, 1 year, 2 years, 3 years, 4 years of follow-up, the patient's injured teeth were successfully replanted with no loosening, no discoloration. Conclusions: For complicated dental trauma with alveolar socket wall fracture, the standardized sequential management and multidisciplinary treatment could achieve good treatment effect through accurate injury analysis and prognosis judgment.

    Successful Rescue of Waiting Patients for Sudden Cardiac Arrest in a Specialist Stomatology Hospital: A Case Report

    Objective: This paper summarizes the rescue process of sudden cardiac arrest of an elderly patient waiting for treatment in a specialized stomatology hospital, providing a reference for the development of emergency systems and procedures for specialized stomatology hospitals and dental clinics. Diagnosis and treatment: A 85-year-old female patient in the stomatology hospital suddenly went into cardiac arrest, the stomatology hospital staff immediately carried out cardiopulmonary resuscitation, activated the emergency plan, used emergency drugs, established intravenous channels, carried out endotracheal intubation, ventilator controlled ventilation and a series of emergency measures. The rescue process was tense and orderly. Results: After 62 minutes of emergency treatment, the patient's heartbeat gradually recovered, and the transfer was successfully completed for follow-up treatment. Conclusions: As a staff in a stomatology hospital, it is important to be proficient in a variety of common medical emergency knowledge and skills, such as cardiopulmonary resuscitation, cricothyroidotomy and emergency drug use, etc. At the same time, it is necessary for each department to allocate emergency equipment and drugs reasonably, develop corresponding rescue systems and procedures, and conduct regular first aid drills.

    A Qualitative Study on the Attitude of Resident Dentists Towards Virtual Simulated Dental Preparation Teaching in General Dentistry

    Objective: To evaluate teaching effect of virtual simulation tutoring system in standardized residency training of dentistry and to improve the application of virtual simulation tutoring system. Methods: Qualitative research method was used in this study. Semi-structured interview was conducted in 12 residents who received standardized residency training in the Department of General Dentistry, Peking University School and Hospital of Stomatology from September 2020 to September 2022. They received virtual simulation tutoring system dental preparation teaching. After the interview, the interview recordings were transcribed, and the transcribed text was analyzed assisted by NVivo 11.0 software. Results: From the perspective of residents, the standardization of operation process, the standardization of evaluation process and the promotion of autonomous learning were the three advantages of virtual simulation tutoring. The difference between the system and clinical practice and the system program were the shortcomings. On the whole, the residents' positive emotional attitudes towards virtual simulation tutoring system were greater than negative ones. Conclusions: Virtual simulation tutoring system is the development direction of intelligent education in the dentistry, and residents have a higher acceptance of this new teaching mode. It is suggested that the system be further improved and optimized in order to effectively improve the teaching quality and promote its application in the future.
