    Effects of Photobiomodulation on Craniomaxillofacial Bone Repair

    Craniomaxillofacial bone defects are with problems such as slow repair rate and limited bone regeneration ability, which often lead to delayed or even non-healing of the defect area, seriously affecting the quality of life of patients. Finding more efficient craniomaxillofacial bone repair strategies has become a key research direction in the field. Photobiomodulation refers to the method of using different light sources to produce beneficial therapeutic effects, which has the advantages of non-invasive, simple operation and high spatial and temporal accuracy, and provides a promising way to accelerate cranio-maxillofacial bone repair. In this review, we systematically summarized the effective parameters of photobiomodulation on craniomaxillofacial bone repair, elaborated the mechanisms of photobiomodulation on craniomaxillofacial bone repair, and summarized the prospects of the current clinical transformation application of photobiomodulation devices, so as to provide reference for realizing efficient and safe craniomaxillofacial bone repair strategies.

    Clinical Strategies for Oral Implant Restoration in Patients with Sjögren's Syndrome

    Sjögren's syndrome (SS) is a systemic autoimmune disease with typical clinical symptoms such as dry mouth, dry eyes, and joint pain. Patients suffering from SS might be accompanied by systemic symptoms and had to take kinds of anti-rheumatic drugs for a long time. This condition might affect the treatment of dental implant restoration. As the increasing requirements for quality of life, SS patients' demand for dental implant restoration also increased. Whether dental implant restoration is feasible for SS patients, how to choose the appropriate timing for dental implants, and how to maintain the restoration effect have become difficult problems for dentists. Currently, there are no relevant treatment guidelines or expert consensus on dental implant restoration for SS patients. Based on our team's years of experience in the diagnosis and treatment of oral diseases in patients with SS, especially in dental implant restorations, the authors reviewed relevant research, in order to provide clinical suggestions for the stomatologists.

    Cracked teeth syndrome is a common disease. This article reviews the potential etiology of cracked teeth syndrome, including dental dysplasia, abnormal bite forces, and iatrogenic factors, and progress in the early diagnosis of cracked teeth syndrome, including clinical symptoms, visual enhancement methods, X-rays, optical imaging, and other new techniques. The aim of this study is to provide support for the diagnosis, prevention and treatment of cracked teeth syndrome.

    Comprehensive Treatment for A Patient with Multiple Rral Diseases: A Case Report

    Objective: This article reports a case of a patient with multiple oral diseases, including anterior tooth avulsion, multiple caries and missing teeth, who underwent a comprehensive dental treatment. Diagnosis and treatment: After comprehensive dental examination, the patient's diagnoses included anterior teeth trauma, caries, wedge-shaped defect, chronic pulpitis, residual roots, chronic periodontitis and dentition defect. After ruling out systemic injuries such as cranial trauma, emergency treatment was performed to control acute symptoms and address the anterior tooth injury, including replantation of the avulsed tooth and fixation of the lateral luxated tooth. During the disease prevention and control period, oral hygiene instruction was provided with plaque control as the core, periodontal treatment to control periodontal infection, root canal treatment to control pulp infection, filling treatment to control carious infection, and extraction treatment for hopeless residual roots and wisdom teeth without opposing arch tooth. During the functional and aesthetic restoration period, teeth undergone root canal treatment and missing teeth were repaired. Regular follow-ups and maintenance treatments were performed during the maintenance period. Results: During 5.5 years of maintenance and follow-up, the replanted tooth was removed due to external root resorption and root fracture in the 4th year and replaced with an implant. The patient had good caries control, and the effects of root canal treatment, periodontal treatment, and implant restoration were stable. Conclusions: For patients with multiple oral diseases, a comprehensive and scientific treatment plan should be developed in consultation with the patient based on a comprehensive examination and evaluation. Standardized treatment focusing on etiology analysis and prevention can simplify laterally avulsion oral disease problems and help achieve and maintain long-term oral health.

    Fixed Bridge Restoration after Apical Surgery for A Bridge Abutment Tooth: A Case Report

    Objective: This article presents a case about a right upper central incisor with chronic apical periodontitis, which achieved infection control, aesthetic adjustment and functional recovery, through personalized comprehensive diagnosis and treatment. Diagnosis and treatment: The chief complaint tooth was the abutment of the old fixed bridge. After the old fixed bridge was removed, the right upper central incisor was treated with microscopic endodontic treatment. After 8 months, there was still lesion after root canal therapy. The over filling, cyst-like change and possible extra-root infection were corrected through apical microsurgery. During the period, regular periodontal initial treatment was performed, and other wedge-shaped defects were filled with composite resin. Functional suitability and aesthetic diagnosis wax pattern was used for aesthetic adjustment and functional adaptation. The patient chose the fixed bridge considering the course of treatment, cost, aesthetics, function and prognosis. Results: The periapical inflammation of the right upper central incisor was controlled, and the function and aesthetics of tooth defect and dentition defect were well restored. Conclusions: Through one-step personalized comprehensive diagnosis and treatment, the communication cost and referral times between different specialties of the patient were reduced.

    Digital Technology for the Restoration of Bilateral Maxillary Cusp Teeth in Oral Comprehensive Treatment

    Objective: To report a case of digital technology for the restoration of bilateral maxillary cusp teeth loss in oral comprehensive treatment. Diagnosis and treatment: On the basis of systematic oral examination and definite diagnosis, the patient received sequential oral treatments, including systemic periodontal treatment, extraction of impacted third molar, and restoration of maxillary bilateral missing cuspid teeth. Among them,was implanted and repaired under the guidance of digital guide plate, andwas repaired with cantilevered all-ceramic bonding bridge. Results: The position of the implants guided by the digital implant guide was consistent with the preoperative design. The appearance and function of the implant restoration and cantilevered bonding bridge were satisfactry. After the treatment, the periodontal health was maintained and the function of restorations were planting. Conclusions: The 3D position of the implant can be accurately controlled by digital implant guide, and the digital impression obtained by intraoral scanning technology can achieve guide therapeutic effect.

    A Case of Comprehensive Dental Care of An Elderly Patient with Maxillary and Mandibular Dentition Defect

    Objective: This case reports the multidisciplinary diagnosis and treatment of an elderly patient with maxillary and mandibular dentition defect. Diagnosis and treatment: The oral and maxillofacial surgery, periodontal, endodontic, and prosthodontic treatments were carried out interleaved according to the requirements. During the disease prevention and control period, the hopeless teeth were extracted. For the teeth with chronic pulpitis and chronic periapical periodontitis, the root canal treatments or root canal retreatments were completed. Periodontal treatment and follow-up were done alongside the whole treatment process. During the functional and aesthetic recovery period, pre-restoration review was conducted first, and the design of removable partial denture was carried out to determine the fixed restoration scheme of the remaining teeth. Then, the pre-restoration treatment and fixed restoration of the remaining teeth were carried out. Finally, the maxillary and mandible removable partial dentures were completed, and regular review and follow-up were conducted during the maintenance period. Results: After multidisciplinary diagnosis and treatment, the periodontal maintenance and the awareness of oral health maintenance of the patient were significantly improved. Teeth extraction sockets healed well. The fixed prosthesis of the remaining teeth were completed, and the patient felt comfortable occlusion. No abnormalities were found in the follow-up examination. For teeth with caries, there was no secondary caries after resin filling treatment. For maxillary and mandible removable partial dentures, the retention were well, and the patient felt comfortable with the occlusal movement without interference. Conclusions: Multidisciplinary comprehensive diagnosis and treatment for elderly patients with oral diseases can help improve patients' compliance and awareness of oral health maintenance, and also achieve satisfactory treatment effect. If more missing teeth in elderly patients lead to unstable occlusal relationship, it is necessary to restore correct and comfortable occlusal relationship through occlusal reconstruction when necessary.

    Objective: This article aims to provide treatment plans for patients with severe periodontitis of stageⅣ grade C, which combined periodontal, endodontic treatment, alveolar surgery, orthodontics, and restorative treatment. Diagnosis and treatment: The patient suffered from severe periodontitis, resulting in anterior teeth displacement, multiple teeth loosening, and individual posterior teeth loss, accompanied by individual tooth decay and mandibular impacted wisdom teeth. After adequate communication, the clinical treatment process includes periodontal basic therapy, filling and root canal treatment of decayed teeth, extraction of severely loose teeth and impacted wisdom teeth, periodontal surgery, orthodontic treatment, implant restoration, and periodontal maintenance treatment. Results: Through these comprehensive oral treatment, the patient's periodontal condition returned to health, the anterior interdental space was closed, the occlusal relationship was improved, and the dentition integrity was restored. Conclusion: The diagnosis and treatment of patients with severe periodontitis requires comprehensive consideration, and good treatment results can be achieved through multidisciplinary cooperation.

    Generalized Periodontitis at Stage III Grade C Treated with Combined Modality Therapy

    Objective: This case reports the multidisciplinary combination treatment of generalized periodontitis at stage III grade C and followed up for 4 years. Diagnosis and treatment: A 52-year-old male patient with a chief complaint of multiple teeth mobility was diagnosed with generalized periodontitis by clinical and radio graphic examinations. With the informed consent of the patient, the patient received periodontal initial therapy, root canal treatment, orthodontic treatment, implant surgery, and supportive periodontal therapy for 4 years. Results: After 4 years of multidisciplinary treatment, the patient's periodontal condition is stable. The occlusion tend to be coordinated and stable, and the missing teeth are restored. Conclusions: Initial and supportive periodontal therapy plays an important role in eliminating periodontal inflammation. Periodontal therapy combined with orthodontic and implant therapy are extremely useful to improve function and aesthetic.

    Multi-discipline Dental treatment of An Elder Patient: A Case Report

    Objective: This case reports the multi-discipline treatment of an elder patient. The disciplines involved in this case were restorative dentistry, endodontics, periodontics, oral surgery and prosthodontics. Diagnosis and treatment: The patient had cross-bite, severe attrition of anterior teeth and loss of occlusal support of posterior teeth. He had loss of vertical dimension. The correct vertical dimension was determined, and the diagnostic casts were mounted based on this vertical dimension. Diagnostic wax-up was created to assess the restorability of the dentition on this vertical dimension. Clear acrylic prep-guide was made to guide tooth preparation. Silicone index was used to make temporary crowns. This was to make sure the final vertical dimension was consistent with the diagnostic vertical dimension. Temporary wire retained acrylic dentures were made. The patient wore the temporary dentures for two months to become accustomed to the temporary dentures. The final dentures were made to this vertical dimension. Results: The cross-bite was corrected, and the dentures were comfortable. Conclusion: Through the multi-discipline approach, the treatment results were good. Oral health of the patient was restored.

    Application of Hierarchical Progressive Teaching Method with Formative Assessment System for General Practice Residency Program in Dentistry

    Objective: To explore the use of hierarchical progressive teaching method with formative assessment system to improve the effectiveness of standardized training for general dentistry residents. Methods: Fifteen residents and ten instructors from September 2021 to September 2022 who participated in the standardized training base of general dentistry in the General Department of Peking University Stomatology Hospital were selected for the study. A questionnaire was used to evaluate the effectiveness of the teaching model and the evaluation system. Results: All residents had good teaching results in 1 year training. More than 85% of the residents and teachers rated the hierarchical progressive teaching of each specialty as “very appropriate” or “appropriate”. Conclusions: The teaching model established in this study, which combines hierarchical progressive teaching and formative evaluation system, has achieved good teaching results in our standardized training for stomatological general residents base and provides a new idea for the teaching reform of standardized dental training.
